Community Health Worker

The CHW is responsible for extending the reach of Community Clinical Services in the local community. The CHW engages community members around their health needs, focusing on cancer screenings and follow-up; identifies and refers patients to other local resources, serves as a cultural broker for program services, supports patients in accessing health care, empowers clients to develop skills for navigating the health system, and teaches clients how to adopt healthy behaviors. The CHW works closely to support other staff responsible for the coordination and delivery of medical services to patients. The majority of work may occur within the community.

Patient Service Representative:

The Patient Service Representative serves as the primary point of contact for patients, acting as an information conduit between the patient and the physician's practice.  They will be responsible for welcoming visitors and patients by verbally acknowledging and offering assistance, inquiring about the reason for the visit and responding appropriately based on needs, arranging for visitors to meet with the necessary personnel, and preparing and providing information and documentation.  Additionally, the Patient Service Representative will be responsible for managing patient appointments, and physician schedules and coordinating follow-up and ancillary services for patients.

Outpatient Counseling

The Clinician position at CCS Outpatient Counseling will provide counseling services in an outpatient setting. The model of treatment provided is dependent upon each individual's needs.  This role will be responsible for collaborating with other providers to provide holistic care to clients, developing individualized treatment plans for each patient that identifies individualized, measurable goals, and documenting treatment progress and goals in an Electronic Medical Record (EMR).  The Clinician will model appropriate social interaction skills, including boundaries, communication, conflict resolution, etc. with patients, families, peers, and others.
